Wondering whether Alpha-Stim might fit?
- Your anxiety has a physical signature your mind cannot talk its way out of.
- You fall asleep and wake at three, and the night ends there.
- You have tried medication and want another option to consider alongside or instead.
- You are already in trauma therapy and the between-session load is still heavy.
- The deeper work would be easier to engage with if the nervous-system noise came down first.
Alpha-Stim is a cranial electrotherapy treatment we offer alongside therapy, before therapy, or on its own. It is used flexibly: some clients use it as needed, others at regular intervals such as daily or every other day. It is not a replacement for the relational work, and it is not a magic shift in a single session. We talk through what you are working with at the consultation and figure out together whether it is a fit.
What is Alpha-Stim?
Alpha-Stim is an electromedical treatment that is FDA-cleared for anxiety and insomnia in the United States, with clearance for depression outside the United States. It uses a low-level microcurrent (cranial electrotherapy stimulation, or CES) delivered through small clip electrodes that attach to the earlobes during a session. The current is well below the threshold of typical sensation, so most people feel little to nothing while it's running.
Alpha-Stim can be used as a stand-alone treatment, alongside ongoing psychotherapy, or as preparation before starting psychotherapy. It is used flexibly rather than on a fixed schedule: as needed, or at regular intervals such as daily or every other day, depending on what you are working with. Many clients combine it with the other work they're doing rather than choosing one or the other.
For more information about Alpha-Stim itself, the device manufacturer publishes research and product details at alpha-stim.com.
How it works
Alpha-Stim delivers a microamperage current through clip electrodes placed on each earlobe. The current is patented and proprietary in its specific waveform, but the overall mechanism is cranial electrotherapy stimulation, a category of treatment with decades of clinical study behind it.
The exact way Alpha-Stim produces its effects is still being mapped by researchers, but current understanding is that the microcurrent influences brain activity in regions associated with mood regulation, attention, and the body's stress response. Functional imaging studies suggest changes in connectivity across networks involved in anxiety and depression.
Sessions are short (typically 20 to 60 minutes depending on the protocol) and can be done while reading, working, or resting. The current is gentle enough that most people forget it is running.
What Alpha-Stim helps with
We use Alpha-Stim to help with anxiety, depression, and insomnia. It is FDA-cleared for anxiety and insomnia in the United States; its clearance for depression is outside the United States, and we use it for depression alongside therapy on that basis. Many clients pursue treatment for one or more of these:
- Anxiety. Decreased anxiety symptoms over the course of regular use, including reduced reactivity to triggers and a quieter overall background state.
- Insomnia. Improved sleep, particularly time-to-fall-asleep and middle-of-the-night waking.
- Depression. A gradual lift in mood over regular use. Depression symptoms tend to take the longest to shift, often three or more weeks, though some people notice change sooner.
Alpha-Stim is not a substitute for psychotherapy when underlying experiences need processing. It does not undo trauma or replace the relational work of therapy. What it can do is reduce nervous-system load enough that the other work becomes more accessible, more sustainable, or both.
How treatment works
Our Alpha-Stim Clinic offers treatment flexibly rather than as a fixed program. Because Alpha-Stim works through the cumulative effect of regular use, most of the benefit shows up over weeks of consistency, whether you use it as needed or at regular intervals such as daily or every other day.
Treatment can run in three different relationships to the rest of your care:
- Alongside ongoing psychotherapy. The most common pattern. Alpha-Stim helps with regulation; the therapy keeps doing the deeper work.
- In preparation for psychotherapy. For some clients, getting symptoms more manageable first makes the therapy itself easier to engage with.
- Without psychotherapy. Some clients use Alpha-Stim as a stand-alone treatment, particularly when symptoms are physiological-leaning and a course of CES is what fits.
Many clients continue using Alpha-Stim on an as-needed basis, sometimes with a personal device they purchased during treatment, adjusting frequency to how they are doing.
The process step by step
1. Consultation
We start with a consultation to determine whether Alpha-Stim is appropriate for what you're working on. In this session, you may have a brief trial experience with the device to see how it feels before committing to the program.
2. Set up treatment
If Alpha-Stim fits, you'll schedule an initial treatment appointment to begin using the device under clinical guidance. After this initial session, you have the option to purchase a device for personal use outside of your therapy sessions. This is something to discuss with your therapist as part of treatment planning.
3. Regular check-ins
You'll have regular check-ins with the prescribing therapist to track progress, adjust the protocol if needed, and address questions. The frequency and length of these check-ins depend on whether you have another therapist involved in your broader care and on your individual needs.
4. Ongoing and as-needed use
As treatment continues, if you have a personal device you can move to as-needed use. If symptoms have shifted in ways that suggest continued treatment, your clinician can talk through next steps. There is no fixed end date; we adjust to how you are doing.
When Alpha-Stim might fit
Some signals that Alpha-Stim might be worth exploring:
- You're managing anxiety, depression, or insomnia and want a treatment option that complements (not replaces) your existing care
- Your symptoms have a strong physiological component, and approaches that work directly with the body and nervous system feel like a useful addition
- You've tried medication and want a non-pharmaceutical option to consider alongside or instead of
- You're already in trauma therapy and could benefit from added regulation between sessions
- You want to reduce the nervous-system load before starting deeper trauma work
Alpha-Stim is not the right fit for everyone. People with implanted electronic devices like pacemakers, vagal nerve stimulators, or cochlear implants should not use Alpha-Stim, and the consultation includes screening for this and other contraindications.

Frequently Asked Questions
What does Alpha-Stim feel like?
Most people feel little to nothing during a session beyond a light tingling at the earlobes. The current is microamperage, well below the threshold of typical sensation. Many clients describe the experience as a settling or quieting that builds over the course of the program rather than a single noticeable shift.
Do I have to be in therapy to do Alpha-Stim?
No. Alpha-Stim can be used alongside ongoing psychotherapy, as a way to prepare for psychotherapy, or as a stand-alone treatment without psychotherapy. The clinical pathway depends on what you're working on. Your consultation will help determine what fits.
Can I take medication and still use Alpha-Stim?
Yes. Alpha-Stim can be used with or without traditional medications. We coordinate with your prescribing provider as needed. We do not adjust medication directly; that remains between you and your prescriber.
Is Alpha-Stim covered by insurance?
Insurance coverage for Alpha-Stim is variable. Some plans cover the service partially or fully; others do not. Contact your insurance provider to verify coverage, or reach out to our intake team and we can help you understand options. The intake consultation can typically be billed through CareFirst BCBS if you have it. See our insurance page for general practice policies.
Can I buy a device for home use?
Yes. After your initial use of Alpha-Stim under a clinician's guidance, you have the option to purchase a device for personal use outside of your therapy sessions. This is something to discuss with your therapist as part of treatment planning. Many clients use a clinic-based start and then continue at home.
How long until I see results?
Alpha-Stim works best with the cumulative effect of regular use. Some people notice changes within the first few weeks; others find shifts develop more gradually over time. It is designed around consistency rather than a quick single result.
Are there side effects?
Most people experience no side effects. Occasionally clients report mild dizziness, brief headache, or skin sensitivity at the electrode site, especially in the first few sessions. Significant or persistent side effects are uncommon. Tell your clinician about anything you notice and they can adjust accordingly.
Who shouldn't use Alpha-Stim?
Alpha-Stim is not recommended for people with implanted electronic devices like pacemakers, vagal nerve stimulators, or cochlear implants. The intake screening covers this. If you have any neurological condition, seizure history, or are pregnant, your clinician will discuss whether Alpha-Stim fits.
